Which application is right for me?
Full Application
For a full piece of equipment — a wheelchair, walker, specialized bed, specialized chair or similar.
- You must have a denial letter from your insurance.
- There must be a valid medical or functional reason for needing the equipment.
- Unfortunately, we can't cover preference requests — for example, wanting a different color or style when the covered equipment meets the need.
Co-pay Application
Insurance is covering part of the cost, but you're facing a large co-pay and need help.
- Apply, and we'll ask you to send us the invoice.
- We'll help you cover the co-pay.
Submitting an application does not guarantee funding. Every request is reviewed individually and depends on available funds.
