Welcome to the website of The ERay Mission, a 501(c)(3) nonprofit organization. By using this website, you agree to these terms. If you do not agree, please do not use the site.

1. Not medical advice

The content on this website is for general information only. It is not medical, legal, insurance or financial advice, and it does not replace the advice of a doctor, therapist or other qualified professional. Always talk with a qualified professional about medical care and equipment needs. We do not recommend, prescribe or endorse any particular equipment, provider or treatment.

2. Applications and funding — no guarantee

• Submitting an application does not guarantee that a request will be approved or funded.
• Every request is reviewed individually. Decisions are made at The ERay Mission’s sole discretion, based on our eligibility guidelines, the information provided, and available funds.
• Full equipment requests require an insurance denial letter and a valid medical or functional reason for the equipment. We are unable to fund preference-based requests (for example, a different color or style).
• Approved funding is paid toward the invoice for the approved equipment or co-pay. We are not the seller, manufacturer or provider of any equipment and are not responsible for its fit, quality, delivery, warranty, maintenance or performance.
• You agree that the information you submit is true and complete to the best of your knowledge. We may decline or withdraw funding if information is inaccurate or incomplete.
• Funding a request once does not create an obligation to fund future requests.

3. Donations

• The ERay Mission is a tax-exempt organization under Section 501(c)(3) of the Internal Revenue Code. Donations are tax-deductible to the extent allowed by law. Please consult your tax advisor about your own situation.
• Donations are used to further our charitable mission. We may direct funds where the need is greatest.
• Donations are generally non-refundable. If you believe a donation was made in error, please contact us promptly and we will review your request.
• Online donations are processed by a third-party payment provider, and its terms also apply.

4. Donor privacy

We do not sell, rent, trade or share donor information. See our Privacy Policy for details.

5. Using this website

Please use this website only for lawful purposes. Do not submit false information, attempt to disrupt the site, or send unsolicited advertising through our forms.

6. Content

The name, logo, text and images on this website belong to The ERay Mission or are used with permission, and may not be copied for commercial use without our written consent.

7. Links to other websites

This site links to forms and pages operated by others (such as our application forms and donation processor). We are not responsible for the content, policies or practices of those sites.

8. Disclaimer and limitation of liability

This website is provided “as is,” without warranties of any kind. To the fullest extent allowed by law, The ERay Mission and its directors, officers, volunteers and supporters are not liable for any indirect, incidental or consequential damages arising from your use of this website or from any equipment, provider or service connected with a funded request.

These terms are governed by the laws of the state in which The ERay Mission is organized, without regard to conflict-of-law rules.
